Agent handoff fails for second draft

This bug happens with every single use of Spiral for me. The orchestrator fails to hand off to the writer whenever making edits. Sometimes it shows the chain of thought but doesn't proceed. Sometimes nothing happens. I have to prompt it in different ways. Sometimes just pasting in my previous message works. Other times I have to encourage it to keep going. Sometimes I've just started over.
